Reviewer Consent to Serve Form

Reviewer Qualifications
Education: a minimum of a BSN preferred
NCNA Member

Preferred experience:
1. Three years of experience as an RN in the related field of interest – prison nursing, nursing home, hospital, school, private practice office or clinic, etc.
2. More than one area of nursing during career
3. Exposure to involvement with a larger system such as: a. staff nurse who served on a hospital-wide committee; b. public health nurse who served on a department wide committee; c. supervisor responsible for more than one area of a facility; d. a nurse who has experience with survey process such as a significant role in preparing for: JCAHO survey or whatever type evaluation that the workplace has; involvement in preparation for Magnet application; someone who has been a surveyor in a state or national initiative.

Additional:

  1. Willing to attend training and agree to promptly review materials, visit the site, and return an evaluation
  2. Able to compare written guidelines and standards with submitted written materials and onsite reviews
  3. Willing to learn
  4. Respectful, considerate approaches to other people such as reviewer team members and work place sites to be visited
  5. Knowledgeable about own type of workplace i.e. expectations about quality of service, funding sources, staff requirements, types of policies, external guidelines/rules/policies
  6. Conducts self professionally
  7. Knows what he/she doesn’t know; is willing to ask questions to clarify how to apply established criteria.
